Printing and Supply of Reservation/Cancellation form (CM-257) Reservation/Cancellation Requisiti on Form in llation size-6 x 8.5 Inch (15.2X 21.5 cm). Paper to be used Maplitho Paper 60 GSM conforming to IS:- 1848/2007 or latest amendment. The form to be printed in Hindi on front side and English on back side in black c olour strictly as per specimen. Binding: Material should be supplied in packing of 1000 Forms in each bundle. The art work for printing and sample workmanship should be got approved from consignee before bulk supply. Packi ng Conditions:- 03-04 bundles of 1000 forms each shall be packed in suitable size of five ply corrugated paper co nstructions carton box conforming to IS: 2771 (part-1) 1990 with amendment No.1 and 2 and style 0620 od :S 64 81: 1971. The weight of carton shall not exceed 15-16 Kgs. approx. The carton shall be than closed with gum pap er tap 50 mm (min.) wide and finally secured by nylon strapping of size 13mm wide 02 nos, each side. The edge of nylon strap shall be heat sealed overlapping each other by min. 25 mm. Packing slip must be pasted on carton box indicating PO No. & other details.
